Keeping Your Child’s Information Up to Date

It is really important that we have accurate and up-to-date information about your child and your family. Please let us know as soon as possible if there are any changes of circumstances that may affect your child’s care, safety, wellbeing or our ability to contact you.

This includes changes to:

1. your home address

2. telephone numbers

3. email address or emergency contact details

4. changes to who is authorised to collect your child

5. changes in family circumstances or parental responsibility

6. changes to your child’s GP, health visitor, dentist, hospital or other healthcare professionals.

 

Please also tell us about any

1. new medical conditions, allergies

2. medications

3. dietary requirements

4. additional needs

5. developmental concerns

6. diagnoses or professional involvement

7.  any changes to your child’s sleep

8. toileting

9. communication

10. behaviour

11. emotional wellbeing or other needs that may help us support them appropriately.

 

We also need to know about significant changes at home, such as

1. a new sibling

2. bereavement

3. separation or other circumstances that may affect your child emotionally.

4. If your child receives support from outside professionals, such as a Speech and Language Therapist, Occupational Therapist, Paediatrician, Educational Psychologist or other specialist service, please keep us informed so that we can work together and provide consistent support.

5. any changes to your child's nursery attendance arrangements

6. collection arrangements or authorised adults

 

Please let us know too. Keeping this information current helps us to safeguard your child, respond appropriately in an emergency and ensure that the care and support we provide reflects their individual needs.
